Sample FY 2024 Dialysis Facility Report
This document contains a sample of the Dialysis Facility Reports that are posted on our secure site. Please note that Table 14 is created only for those facilities that treated at least five pediatric patients over the four year period and Table 15 is created only for those facilities having more than ten patients treated in the facility on December 31, 2022 and were in a nursing home at least one day during 2022.

DFR CODES
The list of  all codes (i.e., ICD-10, HCPCS) used to calculate the following measures reported in the DFR: hospitalizations (Table 4), influenza vaccinations (Table 7),  transfusions (Table 8), Kt/V reported in Medicare claims (Table 9), standardized fistula rate, long term catheter rate and access related infections (Table 11), and prevalent comorbid conditions (Table 12).

This document lists the  ICD-10 codes for prevalent comorbidities used in the Standardized Hospitalization Ratio (Admissions and Days) calculations for dialysis facilities.
